Statement of Nondiscrimination
Blessed Sacrament Catholic Academy admits students of any race, color, national and ethnic origin to all rights, privileges, programs and activities generally accorded or made available to students at the school. It does not discriminate on the basis of race, color, national or ethnic origin in the administration of its educational and admission policies, athletic programs or other school programs.

I. Documentation
A. For All New Registrants

  1. An original birth certificate
  2. A baptismal certificate or dedication certificate (if applicable)
  3. An updated immunization record
  4. Social Security Card
  5. Registration fee
  6. All new registrants are required to provide proof of a current and complete physical examination by a health professional
  7. Proof of residence

B. Grades 2 ? 7 (Grade 8 registration is on a very limited basis)

  1. Copy of most recent report card
  2. Copy of standardized tests
  3. A copy of an IEP if applicable
  4. A letter from the non-public school stating that all financial obligations have been met

II. Registration Procedures

A. Pre-K/Kindergarten

    1. The child must be present at registration for an interview with the prospective teacher
    2. Kindergarten children must return on an assigned day for testing
    3. Parents will be given a medical form from the NYC Department of Health

a) All new registrants are required to provide proof of a current and complete physical examination by a health professional.

  • Accepted students are required to attend an orientation session to be scheduled by the teacher.

 

B. Grades 1 ? 7 (8)

    1. The student should be present at the time of registration
    2. The principal will review the report card and testing results. An interview will be conducted in some cases.

a) If warranted, some students will be required to return for testing or attend a recognized summer school prior to final acceptance.
b) If necessary, students may be required to repeat the present grade.

  1. As above, a medical exam is required

I. Documentacin

A. Matriculas para alumnos nuevos.

  1. Certificado de nacimiento original
  2. Certificado de bautismo o certificado de consagracin (si tiene)
  3. Cartilla de vacunas
  4. Cartilla del Seguro Social
  5. Pago de matricula
  6. Todos los alumnos nuevos tienen que proveer una prueba de un examen fsico
    completo hecho por un profesional de la medicina
  7. Prueba de residencia

B. Grados 2-7 (Grado 8 solo en casos especiales)

  1. Copia ms reciente de la cartilla de notas (report card)
  2. Copia de exmenes estatales
  3. Copia de IEP (si tiene)
  4. Si un alumno se est cambiando de un colegio privado, una carta que indique que se
    han cumplido con todas las obligaciones financieras

II. Procedimiento de Matricula

A. Infantil (Pre-K) Prvulos (Kindergarten)

    1. El nio ha de estar presente cuando se le matricule para una entrevista con su futuro profesor
    2. Prvulos (Kindergarten) han de volver a la escuela en un da asignado para un examen
    3. Se les dar a los padres un formulario mdico del NYC departamento de salud

a) Los matriculados por primera vez se les requiere que traigan un examen mdico
realizado recientemente.

  1. Los alumnos aceptados atendern a una sesin de orientacin con la profesora

B. Grados 1-7 (8)

    1. El estudiante ha de estar presente cuando se realice la matricula
    2. La directora revisar sus notas. En algunos casos se realizar una entrevista.

a) Si el estudiante es considerado candidato, puede requerrsele hacer un examen
o asistir a clases de verano antes de ser aceptado.
b) Si es necesario, se puede pedir al estudiante que repita curso.
